Introduction

This intends to guide you through the process of reporting practitioners (optometrists or dispensing opticians) and facilities (clinics, labs, and institutions) for malpractice and other professional misconduct.

First Step: Search Professional

To report a profile (optometrists, dispensing opticians, clinics, labs, and institutions), you must first find the profile using our Public Search service by clicking Look up a practitioner at the top of this page.

Once you have you will be taken to a page where you have to click on Go to Search which will then bring you up to the search form, type the name, email, or board number of the practitioner or facility into the input box and click search to run the search.

Next, select the right profile from the search results list, this will take you to the profile page. If you cannot find the profile, click here to report a potentially unregistered/approved facility or practitioner.

On this page scroll down until you see the report profile form, fill out the form

Enter your name, and email address, select a reason, and provide a detailed explanation of the report you would like to make, once you are done click the submit button to submit the report.
